IBC Case Manager
The Integrated Behavioral Health Case Manager (SUDS) will provide comprehensive case management and care coordination services to patients/persons with a history of addiction and or suffering from or suspected of substance abuse, to include but not limited to intake assessment, goal setting, long-term care plan development, weekly case plan development, progress monitoring, family and individual advocacy, and referrals. Position will participate in QI activities as needed.
Eligibility Enrollment Specialist/ Referral Coordinator
The Eligibility Enrollment Specialist/Referral Coordinator will provide enrollment assistance and eligibility verification to Rocky Mount Housing Authority (RMHA) residents seeking Medicaid, Medicare, Affordable Care Act (ACA) and other government subsidy care programs. Position will be responsible for identifying barriers to care such as lack of insurance, transportation, etc. and referring resident to appropriate health care and/or social service resources. Position will participate in QI activities as needed.
The Program Manager will be responsible for management, oversight, and coordination of initiatives and activities related to Rocky Mount Housing Authority (RMHA) residents. These duties will include engaging and advising RMHA staff and residents on quarantine and cleaning/disinfecting best practices within the context of the COVID-19 pandemic, providing triage services, conducting health care needs assessments, and providing care coordination to facilitate residents’ referrals to appropriate heath care providers. Position will participate in QI activities as needed.
Jr. Network Administrator
The role of the Jr. Network Administrator is to assist the Information Technology Manager in the administration of all IT infrastructure systems (hosting, network, storage, voice and cabling) in a clinical, office setting. This includes troubleshooting and resolving hardware and software problems in a timely and accurate fashion and providing end user training where required. Good documentation practices will be required for all activities that relate to the configuration and support of these IT systems.
Clinical Administrative Assistant
The Clinical Administrative Assistant serves as the principal administrative support for the Medical Director and clinical team of the organization. The Clinical Administrative Assistant maintains the appropriate files, prepares correspondence and reports, coordinates the flow of internal communication within the Corporation. The Clinical Administrative Assistant is responsible for coordinating administrative activities for OIC Family Medical Center as required by the Medical Director.
Quality Improvement Coordinator – Clinical Administrator (LPN or RN Required)
The Quality Coordinator supports the activities of the Quality Improvement (QI) Program for the clinic. The primary responsibility of the quality improvement coordinator is to ensure that all staff complies with quality, service performance and medical record policies and procedures.
Clinical Medical Assistant (Certification Required; Bilingual Preferred)
Performs Clinical duties as they pertain outpatient clinical care. Assists provider (Physician/Nurse Practitioner/Physician Assistant) with medical procedures and tests in conjunction with patient visits. Position will participate in QI activities as needed.
Nurse Practitioner And/Or Physician Assistant
Responsible for health care coordination for patients in accordance with State and Federal rules and regulations and the nursing standards of care (assessment of health status, diagnosis, development of plan of care and treatment, implementation of treatment plan, and evaluation of patient status). Position will participate in QI activities as needed.
